Replacing a Lost or Stolen Driver’s License

1. Log onto eGov and fill out Application for Driver License/Non Driver ID (Form DS-1972)

2. Attach one (1) recent color passport photograph not more than six months old

To report a change of resident's address:

1. Log onto eGov and fill out Application for Driver License/Non Driver ID (Form DS-1972)

2. Submit a copy of the current U.S. Department of State driver license

3. Submit one (1) color passport photograph, not more than six months old

4. Submit Form DS-2006 showing the new address, if it has not been submitted already

Note: Do not surrender the current license until the replacement driver's license (with the new address) has been issued.
