Expiration Date

This information is usually located at the top right corner of the collection instrument.

But also look for the PRA Statement. The PRA Statement explains to the public, consequences of not displaying correct OMB information on the collection instrument, and an address to send suggestions or questions regarding ways to decrease the burden time. The PRA Statement is sometimes located at the bottom or on the instructions page of the collection instrument.
